Will my insurance cover my visit?
It depends on your insurance. We are an out-of-network provider and if your insurance covers out-of-network providers (call and ask) I can provide you with a superbill following our session. You will give this to your insurance and they will reimburse you directly.
What should I wear?
Wear clothes that you can move in, and I can see you move in. For women, 'yoga' pants are good. For men, shorts work well, or pants that are loose, but not too baggy. I need to see you move, and be able to easily locate your joints. If you have shoulder pain, a tank top would work, and for knee pain either shorts or pants that can be rolled will be fine.
What paperwork do I need?
The beauty of direct access in California is you can see a Physical Therapist directly -- you don't need a Physician's referral on our first visit. After we schedule your evaluation, I will email you a patient intake form and also a liability waiver to peruse, fill out, and sign to bring. Don't worry, I can provide you with copies for you to fill out on our first visit if you prefer.
